Who is the treating dental provider?
Dental care described on this website is provided through Smile Aesthetics and Implant Dentistry Corp. by Marshall Moini, DMD, Florida Dental License No. DN24529. For patients accepted by the practice, Dr. Moini is identified as the dentist of record and the clinical record identifies who performed each service.
Who makes clinical decisions?
Dr. Moini exercises independent clinical judgment. Marketing personnel, clerical personnel, financing companies, and technology vendors do not diagnose patients or determine whether a procedure is appropriate.
Who may see my dental records?
Access is limited to authorized workforce members and others permitted or required by law for treatment, payment, health care operations, patient-authorized purposes, and other legally recognized uses. The practice’s HIPAA Notice of Privacy Practices explains these uses and your rights in greater detail.
Can I request a copy of my records?
Yes. Submit a records request using the process provided by the office. Identity and authority may be verified. The practice will respond within the time required by applicable law and may charge only a legally permitted, reasonable, cost-based fee.
Are consultations confidential?
Patient information is handled under applicable privacy and confidentiality requirements. Complete privacy cannot be guaranteed in ordinary public areas, standard email, or consumer messaging services. Ask the team for a private or secure communication method when needed.
What document controls the treatment and price?
The patient-specific written treatment and financial agreement controls. Website descriptions, preliminary conversations, advertisements, examples, estimates made before examination, and general price discussions are not final treatment contracts.
Does financing approval mean treatment is appropriate?
No. Financing is separate from diagnosis. A lender’s approval concerns credit and does not establish clinical candidacy. Treatment is recommended only after the appropriate examination and diagnostic evaluation.
Are results guaranteed?
No. Dentistry involves biological healing, patient-specific anatomy, materials, functional forces, compliance, and other variables. Simulations, photographs, examples, and treatment goals do not guarantee an outcome.
Can I submit a legal or privacy concern through this website?
Do not send confidential legal communications, medical records, photographs, identification, or sensitive health information through ordinary email. Call 954-852-2830 or email info@saidentistry.com to request an appropriate secure communication method.
How do I raise a complaint?
Contact the office promptly so the concern can be reviewed. Privacy complaints may also be submitted as described in the HIPAA Notice. The practice will not retaliate against a patient for making a good-faith privacy complaint.
